My PAN Card has the wrong Date of Birth listed on it. I downloaded the form 'https://tin.tin.nsdl.com/pan/correction.html'. However, this form does not have any field mentioning the change in Date of Birth.
Please let me know how I can proceed with the rectification of my DOB information in the PAN Card.

Please fill Point No 3 of "Request for New PAN card Or/And Changes or Corrections in PAN Data" Form stating clearly your correct date of birth & attach proof of your date of birth with this form.Please obtain an acknowledgement copy of submission of this Form with Department on Photocopy of this Form for your record.
