my name mitesh sureshbhai patel my PAN no is ALLPP0087G I have no photo copy and any other avidance so pls i can proceed 2nd time but in filling my form miteshkumar instead of mitesh writting so i am agree for as per ITD's Database thanks..........
My PAN Card has the wrong Date of Birth listed on it. I downloaded the form 'https://tin.tin.nsdl.com/pan/correction.html'. However, this form does not have any field mentioning the change in Date of Birth.
Please let me know how I can proceed with the rectification of my DOB information in the PAN Card.

Please fill Point No 3 of "Request for New PAN card Or/And Changes or Corrections in PAN Data" Form stating clearly your correct date of birth & attach proof of your date of birth with this form.Please obtain an acknowledgement copy of submission of this Form with Department on Photocopy of this Form for your record.
Hv a pan card issued in chennai and ive moved to mumbai. ive been filing return for past 2 yrs with mumbai address. now this year i moved back to chennai. can i file the return with chennai address. what formalities need to be taken?

No formalities required. U can simply go ahead & file ur return in Chennai with chennai address

first lodge complaint with police station and then again apply for PAN card quoting the same PAN number as earlier.

I don't think FIR is equired. You have to apply again and DO NOT tick any of the boxes on tmhe left. You will be given a duplicate PAN card with old data
My name 'Girish' is printed as 'Girishchandra' on PAN card. I want to have it corrected to maintain consistency in name across all important documents.How do I go about having it corrected?

again apply for fresh PAN card quoting same PAN number with correct name along with proof of identity enclosing therewith old PAN card.

Dear Girishbhai, oh sorry Girishchandra , oops sorry Girish -- you need to make an affidavit from a public notary saying that you are one and the same person known by different names. Than with that affidavit make an application for new pan/change of name in old pan.
Is it compulsory to file IT return for a housewife having Demat account and investment in Mutual fund and yearly income less than Rs 20,000 Pl Suggest.

obligation to file return arises when income exceeds limit of income not chargeable to tax.
