Nobody joins an organization to screw it. Similarly no organization hires a person to screw him/her. Cos make selections after carefully going through the resume, personal interview, group discussions, etc. Therefore basically there may not be anything seriously lacking in an employee.
If a person doesn't perform fully, there are reasons for it. Discuss with the employee in private/confidence, you will get the answer. Once whatever possible are addressed to, definitely the employee will improve in his/her performance. There will be very few chances for anyone getting fired.

but if company enviornement is not good then ,,,,what a boss can do . Can he change company's enviorment for an employee
Waht about a secretary's or a PA's fate? many times we find ourselves very busy while the PA to one of our big boss sits alone waiting for the master's guidelines. He shares some work given but not like us . should he be fired or it should be left to the boss?

A PA's role is different from a normal employee. They are hired to help the Boss, it may be both official, as well as personal. Personal like keeping track of his Bank accounts, car servicing, purchases, school fees of his children, maintaining files of the Boss insurance policies, payment of premiums, etc. This is because the Co wants the Boss to be taken care of in all these, so that he can work for the organization without any personal problems. The normal employees do not do such personal works of the boss. You may think the PAs don't have any work load, whereas they may doing all the above mentioned works, which you may not be familiar with. In addition, they may be doing some official works as well, as assigned by the Boss, like making his travel arrangements, co-ordination/followup with other Departments, etc. Whether they are good/efficient in handling all these activities or not, is best to be decided by the Boss only.
