PAN related work is outsourced to UTI and NSDL. You must get an "application for change in details" from either UTI or NSDL and attach fresh photograph and other details and also mention your PAN number to get new PAN card with the same PAN number

How to get yourself a PAN card
by Joga Rao on Jan 15, 2005 03:10 PM  Permalink  | Hide replies

In the Do's & Donts - you said use your maiden name and in the text you said women irresepective of marital status must give their father's name. You are not clear there. The correct position is, women need not give their maiden name, they can give their changed famly name / surname if they so want. But they should give their father's full name. In our country usually they give option of father / husband's name in case of ladies. Whereas IT Dept does not accept husband's name. Father's name has to be mentioned whereever required.

Do not waste time in following up an application filed 2 years back. Make fresh application.

NRIs need a local address to apply for a PAN card. A photocopy of passport as proof of identity and some other proof related to the residence address mentioned in the form is required. The processing fee is Rs.60
